running totem from the command line on a file causes various fonts to
be set to a smaller size, and changes mouse settings. for people who
cannot adapt to the new settings, it requires restarting xorg. this
is an accessibility issue for some people.

This must be because totem starts gnome-settings-daemon. As of
gnome-control-center/capplets-data 1:2.20.3-2, the settings daemon will
not be spawned automatically through dbus, so this should be fine now.
Could you confirm this fixes your issue?

I would guess totem talks to gnome-settings-daemon over DBus which
triggers it's startup; gnome-settings-daemon will apply the GNOME Gtk+
settings to your X session (via XSettings) on startup. When you're
using GNOME apps, they will use GNOME preferences; here the problem is
that this affects all Gtk+ programs you already launched.
I can't tell exactly where the logic flaw is, but I don't see this
being fixed any time soon; I'd recommend you configure the GNOME
settings properly.

Are you still experiencing this bug with Totem 2.22 and/or
gnome-control-center/capplets-data version 1:2.20.3-2?
I'm guessing this was caused by the media player keys plugin starting
gnome-settings-daemon, it should be fixed in 2.22.
