Do you have a separate /usr partition? If so, is it a normal
partition or used on top of LVM/RAID/...?

> Please, let me know what other information may be usefull.

Can you please execute:

  bash -x mkinitramfs -o /dev/null >/tmp/trace.log 2>&1

and provide us the resulting /tmp/trace.log file?

/dev/xvda2 does not use LVM inside the virtual machine, but on the
hosting machine, it's an LV at /dev/${vg}/${hostname}-disk.

Attached the (compressed) output from "bash -x /usr/sbin/mkinitramfs

Hello,
 downgrade initramfs-tools package from 0.117 on 0.116 could help.

I believe this was caused by:
http://anonscm.debian.org/cgit/kernel/initramfs-tools.git/commit/?id=be18fee1bd52

The readlink command in initramfs doesn't seem to support -f:

$ readlink -f /
-f: No such file or directory

That depends on whether you use klibc-utils or busybox:

$ /usr/lib/klibc/bin/readlink -f /
-f: No such file or directory
$ busybox readlink -f /
/

I wish we didn't try to support both.

I installed busybox, run update-initramfs -u, restarted the computer

No separate /usr, no LVM or RAID.

Inspired from:
https://bugs.debian.org/cgi-bin/bugreport.cgi?bug=762870#42

Have had the same symptoms on one machine. Installing busybox works
around this issue and boots as expected, though.
