1. You should not increment Debian revision before uploads.
   Just add entries to first changelog like below.

2. You should add "Uploaders: " field to debian/control to enable
   team upload

3. Please describe "How you have done with it" to changelog

>  * Fix W:hardening-no-relro
>  * Fix I:hardening-no-fortify-functions
>  * Fix I:description-synopsis-might-not-be-phrased-properly
>  * Fix X:package-contains-no-arch-dependent-files
>  * Fix I:duplicate-long-description

  Those entries are useless, I suggest you to describe like this...

  * debian/control
    - fix "duplicate-long-description" lintian warning
  * debian/rules
    - add foobar to fix lintian warning "hardening-no-relro" and
      "hardening-no-fortify-functions"

4. Update Standards-Version

 Please, fix those and upload again (newer upstream version is okay, add
 close entry to first changelog entry).

As you pointed out, I've added more detailed entry to changelog.

   Please describe more about copyright holder, contact mail address, URL, etc.
   e.g. if it's a company name, provide it with "foobar, Inc. <contact@foobar.example.com>"

 - You should write about LGPL-2.1 outline text, instead of just putting 
   "See /usr/share/common-licenses/LGPL-2.1 for the full license text."
