Using the PulseAudio output plugin (in plugins-extra, with pulseaudio
0.9.15-4.1), every time the playlist advances, volume decreases.

Using the PulseAudio volume control I just saw 64% -> 31% -> 0, 80% ->
48% -> 15% -> 0%, 100% -> 67% -> 35% -> 2%: call it a decrease of
about 32 volume percentage points each new track).  This happens
during normal playlist advance, when selecting a new song, or when
pressing "forward to next song", or "audacious2 -f" etc.

I have not seen this happen with ALSA plugin, but ALSA output has its
own problems so it is a poor workaround...

PulseAudio output plugin has been removed upstream (in part because of
this very problem) in favor of outputting to PulseAudio through ALSA.
